Try out our system free of charge for 2 months.  For the physician office/clinic setting, the cost per clinician is $75 per month. In the hospital setting, the cost is $1 per completed survey.  Minimums apply.  Additional charges may also apply if you choose to collect information through phone calls or postcards. Above charges assume that comments are collected electronically. For more details contact us.
